Cigna, St. Francis Hospital form collaboration pact

Bloomfield insurer Cigna and St. Francis HealthCare Partners said Monday they have formed a new collaborative relationship that aims to improve health outcomes and lower costs.

The new collaborative accountable care organization is one of many Cigna has been forming with medical providers across the country.

The program will benefit about 20,000 individuals covered by a Cigna health plan who receive care from St. Francis HealthCare Partners’ 216 primary care physicians, 530 specialists and 243 mid-level providers.

Under the program, St. Francis will monitor and coordinate all aspects of an individual’s medical care. Patients most likely to see the immediate benefits are those who need help managing chronic conditions, such as diabetes, heart disease and obesity.

Registered nurses employed by St. Francis will serve as clinical care coordinators and help patients with chronic conditions, or other health challenges navigate the health care system.

The care coordinators will use patient-specific data from Cigna to help identify patients being discharged from the hospital who might be at risk for readmission, as well as patients who may be overdue for important health screenings or who may have skipped a prescription refill.
