HealthyCT inks deal for care-management tools

HealthyCT has signed a deal with a Virginia health-care management firm that will provide Connecticut’s newest nonprofit health plan with software tools for improving patient outcomes and reducing avoidable costs.

As part of the agreement, Altruista Health Inc. will make available to HealthyCT its GuidingCare care management software that will include predictive modeling, gaps-in-care analytics, care, utilization, appeals and grievances management, and role-based portals for providers and member services teams.

The software also provides health risk, condition, transition of care assessments as well as integrated care plans, which will allow HealthyCT to better manage the health of their member population.

Terms of the deal were not disclosed.
