Guilford-based Visiting Nurse Association Community Healthcare & Hospice has merged with Yale New Haven Health, the system announced Monday.

The agency will become part of the YNHH’s regional home-health agency, Home Care Plus Inc.

About 500 employees, most of the agency’s staff, joined YNHH as part of the merger from both the Guilford and Hamden locations.

Terms of the merger were not disclosed.

Kathleen O’Leary, vice president of post-acute care services at YNHH, said, “VNA Community Healthcare & Hospice has long been known for its excellent patient and whole family care that aligns with our mission and vision.”

“In order to build scale and provide the full continuum of care for our patients, YNHHS has looked to partnerships with strong organizations who provide outstanding care in  this evolving landscape,” YNHH said in a statement.

VNA Community Healthcare & Hospice, founded in 1910, provides medical home care in 35 towns. According to its tax forms, the agency lost $708,043 in 2020 on total revenue of $29,883,652, compared to a $218,538 in profit on $30,727,975 in total revenue for the previous year.

VNA Community Healthcare & Hospice Board Chair David Cowan said, “We are excited to join Yale New Haven Health and their mission to provide excellent patient-centered care both in and out of the hospital environment. Our goals are very much aligned, and we look forward to continuing to support our population who want to age in place by providing the critical support services they have come to rely on.”

In 2020, YNHH acquired New Haven-based VNA of South Central Connecticut, citing a goal of expanding its home-health services. The system now operates Home Care Plus subsidiaries in Hamden, Guilford and Waterford.
